4K a year Family Service League (FSL) helps individuals, children, and families to mobilize their strengths and improve the quality of their lives at home, in the workplace, and in the community.
With over 60 social service programs in more than 20 locations and committed staff and support personnel of approximately 700, we continue to serve Long Island's most vulnerable citizens.
Family Service League is seeking a full-time Social Worker for their Assertive Community Treatment Team in Bay Shore, who will deliver services to individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional services.
Responsibilities:
Provide psycho-education to families and clients Conduct daily home visits Write treatment plans Complete client assessments, relapse prevention plans etc.
Correspond with managed care companies, hospitals and community resources Provide motivational interviewing Provide evidence based practice treatment to clients with severe and persistent mental illness Connect client to treatment facilities Conduct crisis interventions Facilitate groups Cover the crisis phone
Qualifications:
Master's degree required.
LMSW/LCSW/LMHC required.
SIFI preferred.
At least one year of experience working with the severe and persistent mental illness (SPMI) population required.
Excellent interpersonal and organizational skills required.
Candidate must possess the ability to multitask, prioritize in a fast-paced environment, and work independently, as well as part of a multi- interdisciplinary team.
Strong communication skills, both verbal and written required.
Proficient computer skills, including Microsoft office/ electronic health records/ documentation required.
Valid and clean New York State Driver's License required.
